Ulcerative Colitis Nursing Care Plan

Ulcerative colitis is an inflammatory bowel disease, which causes long-lasting inflammation and ulcers in the digestive tract. It is a long-term chronic condition with symptoms usually developing over time, rather than suddenly. It can be debilitating and sometimes can lead to life-threatening complications such as bowel cancer or primary sclerosing cholangitis.

While it has no known cure, treatment can greatly reduce the effects of the disease and even bring about long-term remission. The exact cause of ulcerative colitis is unknown, although it is thought to be the result of a problem with the immune system. It is thought that the immune system mistakes harmless bacteria inside the colon for a threat and attacks the tissues of the colon, causing it to become inflamed.

The Care Plan sets out a clear explanation of the resident’s issue, and will guide the nurse or carer through the process of preparing a comprehensive, individual person centred Care Plan.
