Communication Difficulty Nursing Care Plan

Communication difficulties can be particularly frustrating for elderly people and lead to feelings of depression and isolation. Nurses and carers should demonstrate patience and provide the resident with alternative methods of communication when possible, such as using lip reading, sign language, body language, or writing down what he, or she wants to say.

Dementia is one of the major causes of communication difficulty, and can include difficulty in forming whole sentences, or recalling words or phrases. Other medical conditions, such as strokes and Parkinson’s disease can also lead to communication difficulties, resulting in a total inability to communicate, or a decreased ability to communicate effectively. Poor hearing can also cause problems with communication.

Residents with communication difficulties, which are related to dementia, strokes and Parkinson’s disease, can also have comprehension difficulties which should be assessed as early as possible in order to maintain an optimum level of care.

The Care Plan sets out a clear explanation of the resident’s issue, and will guide the nurse or carer through the process of preparing a comprehensive, individual person centred Care Plan.
