Falls Risk Nursing Care Plan

Falls represent the most frequent cause of injury in the elderly population. This can be due to one or more risk factors. The more risks present the greater the risk of falls. Older people are more likely to have a fall because they may have balance problems and muscle weakness, poor vision and a health condition, such as heart disease, dementia or low blood pressure, hypotension, which can lead to dizziness and a brief loss of consciousness.

On admission a falls risk assessment and questionnaire must be completed. It incorporates information from the pre admission enquiry form, Care Needs Assessment Plan, the resident and their relative. This must be reviewed monthly. Many elderly residents prefer to wear slippers for comfort, however, it is of utmost importance that they wear appropriate well fitting and supportive footwear when mobilising to minimise the risk of falls.

The Falls Risk Care Plan sets out a clear explanation of the resident’s issue, and will guide the nurse or carer through the process of preparing a comprehensive, individual person centred Care Plan.
