Palliative Care Plan

Palliative care is the active holistic care of patients with advanced progressive illness. Palliative care treats people suffering from serious and chronic illnesses such as cancer, cardiac disease such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), kidney failure, Alzheimer’s, Parkinson’s and many more.

Management of pain and other symptoms and provision of psychological, social and spiritual support is paramount. The Care Plan sets out a clear explanation of the resident’s issues, and will quickly guide the nurse or carer through the process of preparing a comprehensive, individual person centred Care Plan.
