Pulmonary Embolism Care Plan

A pulmonary embolism is a blood clot in the pulmonary artery, which is the blood vessel that transports blood from the heart to the lungs. It is a serious and potentially life-threatening condition as it can prevent the blood from reaching the lungs.

The main symptoms include chest pain, a sharp stabbing pain that may be worse when breathing in, shortness of breath which can come on suddenly or develop gradually and coughing which may include coughing up blood or mucous containing blood, anxiety, feeling faint or dizzy, or sudden collapse.

The Care Plan sets out a clear explanation of the resident’s issue, and will guide the nurse or carer through the process of preparing a comprehensive, individual person centred Care Plan.
